This Glasgow Coma Scale calculator totals the three GCS components — eye (E), verbal (V) and motor (M) responses — into the 3–15 score, its severity band, and the component breakdown that charts actually record.

Three Responses, One Number — and the Breakdown Matters More

Worked Examples

  • Alert patient, chatting normally, follows commands: E4 V5 M6 = GCS 15 (mild band’s ceiling) — the only combination that makes 15
  • Post-fall patient: opens eyes to pain (E2), groans only (V2), abnormal flexion (M3) = GCS 7 — severe band, the coma/airway-action territory the mnemonic warns about
  • Same total, different patients: E3 V3 M4 = 10 and E2 V4 M4 = 10 share a number but not a picture — why every chart entry writes the components (E/V/M) beside the sum

Scale Discipline

  • Always the best response per axis: a localizing left arm outranks a withdrawing right one.
  • Pain stimulus has a correct technique (trapezius squeeze, supraorbital pressure) — peripheral nailbed pressure misleads motor grading in spinal injury.
  • Sedation, paralysis, intubation, alcohol and post-ictal states all bend the scale — document confounders, and repeat the score: trend beats snapshot.
  • A pediatric modification exists for pre-verbal children (verbal axis re-scored) — this is the standard adult scale.

Frequently Asked Questions

How is the Glasgow Coma Scale scored?

Eye response 1–4 + verbal response 1–5 + motor response 1–6 = 3 to 15. Best eye: spontaneous 4 … none 1. Best verbal: oriented 5 … none 1. Best motor: obeys 6, localizes 5, withdraws 4, abnormal flexion 3, extension 2, none 1. This calculator sums your selections and shows the components.

What GCS is considered severe?

13–15 is mild, 9–12 moderate, and ≤ 8 severe — the coma range associated with inability to protect the airway, source of the “GCS 8, intubate” teaching phrase. Bands guide urgency; trends and components guide management.

Can a GCS be 0?

No — the floor is 3 (E1 V1 M1). A recorded “GCS 0” is always an error; even a patient with no responses in any domain scores 3. This matters downstream in trauma registries and prognostic models that treat 3 as the minimum input.

How is GCS scored in an intubated patient?

The verbal component can’t be tested through an endotracheal tube: it’s recorded as “T” (e.g., E3 VT M5) and totals are often estimated from eye + motor (or a derived verbal estimate). Never silently score V as 1 — that fabricates three points of deterioration.

What is the best motor response to record?

The best response from any limb. GCS grades the patient’s best overall performance per component — asymmetric findings are documented in the neuro exam, but the scale takes the top score for prognosis comparability.

Is this GCS calculator medical advice?

No — an educational implementation of the published scale (Teasdale & Jennett, 1974). Neurological assessment belongs to trained clinicians at the bedside.
